Martin Blueberries at Columbia Station bills itself as Cleveland's u-pick blueberry farm, and geographically it has a case: under thirty minutes from Medina, fifteen from Strongsville, forty from the city.
The farm also grows chestnuts, per the county directory.

Closed Mondays in season. The picking window is a morning one and it is short: four hours a day.
Pro tip: an 8am to noon window and a mid-June start means the good picking here is over before most people have thought about blueberries. Diary it for the first week of June and watch the website, because by the time the season feels like blueberry season this farm has usually shut.
